What is a ketogenic diet? What the science says
 This guide is relevant for people with any type of diabetes. I will mainly talk about insulin when I discuss how a keto diet affects blood sugar, but some studies also show a possible reduction in certain type 2 medications. Disclaimer: Please always consult with your medical team before you start a new diet, adjust your medication or change your diabetes management routine. What is a ketogenic diet? Once upon a time, keto was the original “diabetes diet” prescribed to type 1 diabetes patients before the advent of insulin, as this would prolong their lives as it has less of an impact on blood sugar levels. 5% of your daily caloric intake from carbohydrates. By restricting your carbohydrate intake so severely, you force your body to get most of its energy from fat. A byproduct of this fat burning is the production of natural ketones in the body, hence the name of the diet. Burning ketones supplies the body with an alternative form of energy rather than quickly accessible energy from carbs (glucose) and is what makes the ketogenic diet work. IMPORTANT: natural ketones are different from the “bad” ketones that can lead to diabetic ketoacidosis (DKA).|People with diabetes who follow a keto diet need to monitor their ketone levels carefully. If levels get too high, ketoacidosis can occur. DKA is a condition wherein the levels of ketones become extremely high, poisoning the body. It is a severe and dangerous condition that can develop rapidly, sometimes within the space of 24 hours. There are several potential triggers for ketoacidosis. However, it most often occurs due to illnesses that cause higher levels of hormones that work against insulin. It can also occur in people with type 2 diabetes, though this is much less common. High levels of ketones in the urine and high blood sugar levels are both signs of ketoacidosis. A person can test for ketoacidosis using a kit at home. Research shows that cutting off glucose levels with a very low-carb diet makes your body produce ketones for fuel. This change can help to reverse neurological disorders and cognitive impairment, including inducing seizure control. The brain is able to use this alternative source of energy instead of the cellular energy pathways that aren’t functioning normally in patients with brain disorders. A related clinical diet for drug-resistant epilepsy is called the medium-chain triglyceride ketogenic diet, in which MCT oil is extensively used because it’s more ketogenic than long-chain triglycerides. 13) Another dietary therapy for epilepsy called Low Glycemic Index Treatment (LGIT) was developed in 2002 as an alternative to the keto diet. Clinical improvement was observed in Alzheimer’s patients fed a ketogenic diet, and this was marked by improved mitochondrial function. 15) In fact, a European Journal of Clinical Nutrition study pointed to emerging data that suggested the therapeutic use of ketogenic diets for multiple neurological disorders beyond epilepsy and Alzheimer’s, including headaches, neurotrauma, Parkinson’s disease, sleep disorders, brain cancer, autism and multiple sclerosis. Because most folks live on a high carb diet, our bodies normally run on glucose (or sugar) for energy. We cannot make glucose and only have about 24 hours’ worth stored in our muscle tissue and liver. Once glucose is no longer available from food sources, we begin to burn stored fat instead, or fat from our food. Therefore, when you’re following a ketogenic diet plan for beginners, your body is burning fat for energy rather than carbohydrates, so in the process most people lose weight and excess body fat rapidly, even when consuming lots of fat and adequate calories through their daily food intake. Another major benefit of the keto diet is that there’s no need to count calories, feel hungry or attempt to burn loads of calories through hours of intense exercise. In some ways, it’s similar to the Atkins diet, which similarly boosts the body’s fat-burning abilities through eating only low-carb foods, along with getting rid of foods high in carbs and sugar.|When we eat more carbohydrates than we need, our body stores them for later use. Stored glucose is packed tightly into molecules called glycogen which can be used to maintain our body in case we go too long without eating a meal. These reserves can only last about one day; and once depleted, the body must switch to a different fuel source called ketones. Ketones are produced in the liver. In response to low levels of glucose, the body begins breaking down fat tissue for fuel in the form of free fatty acids. Free fatty acids can be used as a fuel source throughout the body except for in the brain. The chart above indicates that the keto diet requires eating macronutrients at a ratio of 60% fat, 30-35% protein and 5-10% carbs. This causes the brain to hit the metabolic panic button in demand for a source of fuel. In response, the liver begins to repackage fatty acids into ketones, which are allowed access into the brain for fuel. Removing glucose from carbohydrate foods will cause the body to burn fat for energy instead. The major differences between the classic keto and the Atkins diet is the former emphasizes healthier keto fats, less overall protein and no processed meat (such as bacon) while having more research to back up its efficacy. In fact, those differences with Atkins outline some of the popular keto diet myths, such as it being another high-protein plan, recommending any type of fat and that barely any science research backs up the benefits. These are nutrition lies, plain and simple. So is the keto diet healthy? If it’s done Atkins style? No. But if relying on healthy fats, greens and organic meats? What does “keto” stand for exactly? Following a ketogenic diet puts your body into a state of “ketosis,” which is a metabolic state that occurs when most of the body’s energy comes from ketone bodies in the blood, rather than from glucose from carbohydrate foods (like grains, all sources of sugar or fruit, for example). Additionally, some patients may feel a little tired in the beginning, while some may have bad breath, nausea, vomiting, constipation, and sleep problems. Is a ketogenic diet healthy? We have solid evidence showing that a ketogenic diet reduces seizures in children, sometimes as effectively as medication. Because of these neuroprotective effects, questions have been raised about the possible benefits for other brain disorders such as Parkinson’s, Alzheimer’s, multiple sclerosis, sleep disorders, autism, and even brain cancer. However, there are no human studies to support recommending ketosis to treat these conditions. Weight loss is the primary reason my patients use the ketogenic diet. Previous research shows good evidence of a faster weight loss when patients go on a ketogenic or very low carbohydrate diet compared to participants on a more traditional low-fat diet, or even a Mediterranean diet. However, that difference in weight loss seems to disappear over time. A ketogenic diet also has been shown to improve blood sugar control for patients with type 2 diabetes, at least in the short term.|For someone eating 2000 calories per day, that translates to around 167g fat, 75-100g protein, and 25-50g carbohydrates. This is key: the total calories you consume are not NEARLY as important as the overall ratio of protein/fat/carbohydrate that you take in. Consuming foods with this macronutrient ratio causes our bodies to burn fat for fuel instead of carbohydrates, meaning it’s a simple (not easy, but simple) way to burn body fat more efficiently. Because of this, the ketogenic diet has been getting a ton of buzz recently for its weight loss benefits. Many people who adopt a ketogenic lifestyle tout easy weight loss from following the protocol, without adding in extra exercise or worrying about total calories. It’s true, you can experience rapid, significant weight loss if you follow this diet correctly, to do this safely, there are other nutritional considerations - more on that soon. WHERE DID KETO COME FROM? The ketogenic diet is the gold standard diet for epilepsy, and research demonstrates that these benefits could actually extend to other neurodegenerative diseases as well. In fact, the keto diet has serious risks. For one thing, it’s high in saturated fat, which has been linked to heart disease. Additionally, a nutrient deficiency and constipation could occur since the keto diet is very low in fibrous foods such as fruits, vegetables, and whole grains. Liver problems for those with existing liver conditions could possibly worsen since the keto diet puts stress on the liver, and kidney problems could also occur. If you are considering going on the keto diet, work with a knowledgeable practitioner or seek out a registered dietitian with experience in prescribing it and following you to avoid any adverse effects, Dr. Goss says. Maintaining proper electrolyte balance is important while following a keto diet. Depending on a person’s activity levels, it is recommended that 2 to 7 grams of salt be added to the diet daily. That means choosing omega 3-rich foods, like wild salmon, grass-fed, local, or organic meats, and snacking on whole foods rather than processed keto-approved treats. It also means trying to follow a balanced diet as best you can by incorporating as many nutrient-rich fruits and veggies as you can. Many registered dietitians aren’t a fan of keto because it may lead to nutrient deficiencies. Overall, more research is necessary. For the general population, is the keto diet healthy? While this diet has been shown to accelerate weight loss and to improve certain health conditions, it has its fair share of critics. At that point, the hurdles start to feel insurmountable, she says. What’s the healthiest way to try keto cycling? Despite the concerns, if you want to give keto cycling a try, experts recommend cycling in healthful sources of carbs instead of those craveable, heavily processed refined or sugary carbs. Think sweet potatoes, beans, milk and fruit over muffins, rolls and chips. It’s also a good idea to make a plan for your carb-heavy days so you get the most benefit. For example, Spritzler says you might eat them on a workout day. “Pay attention to how you feel on the days you eat more carbs,” she says. Some questions she suggests asking yourself: “Do your energy levels drop or soar? Are you happier and satisfied or anxious and hungry? Does your sleep improve or worsen? ” Pay attention to your answers. “If you feel better off eating carbs occasionally and it doesn't seem to interfere with your weight, keto cycling may be a good fit for you. The ketogenic diet essentially forces the body to use fat as its main source of energy instead. It does this by mimicking a state of starvation where the body breaks down fat stores and converts them into ketones through a biochemical process called ketosis. Reaching ketosis: Generally speaking, the ketogenic diet is composed of 70-75 percent fat, 20-25 percent protein, and 5-10 percent carbohydrates. There is no one standard ketogenic diet, and different ratios of nutrients have been used in so-called keto diets. All have in common the reduction of carbohydrates and an increase in fat along with a moderate amount of protein. The reduction in carbohydrates deprives the body of glucose and causes a metabolic state known as ketosis, due to the accumulation of molecules known as ketones in the bloodstream. Ketones consist of acetoacetate, acetone, and beta-hydroxybutyrate and form in the liver from long- and medium-chain fatty acids when the body burns stored fat for energy after glucose is depleted or in situations in which there is inadequate insulin present for glucose to be used as energy. In addition to seizure disorders, ketogenic diets have been tested in the management of some people with other conditions including diabetes, cancer, polycystic ovary syndrome, and Alzheimer's disease. Further, the "keto diet" has gained attention as a potential weight loss tool. Its proponents argue that a carefully controlled ketogenic diet can avoid the dangers of ketoacidosis and be an effective way to lose weight. It’s been around for nearly a century, and has its roots in the medical world: In the 1920s, epilepsy researchers found that increased levels of ketones in their patients resulted in fewer seizures, and the diet is still a widely accepted treatment for epilepsy today. There’s also some evidence that a ketogenic diet has therapeutic potential for a wide array of symptoms and diseases, including cancer, polycystic ovary syndrome, neurological conditions, diabetes, and even acne. So how did it become a trendy weight-loss diet? Though keto has been gaining popularity recently thanks to celebrity endorsements and a wealth of diet-adherent Pinterest recipes, this is not the first time a form of it has gone mainstream. “We have been seeing different variations on the low-carb diet for decades,” says Abbey Sharp, a registered dietician and nutrition blogger. “The Atkins diet and the Dukan diet are two variations of a ketogenic diet.

The ketogenic diet can be dizzyingly complicated. You want to load up on fats and protein, and keep your carb intake low-but all fats and proteins aren’t alike, and there are some veggies higher in carbohydrates than others. Oh, and fruit is pretty much banned. But don’t worry: We’ve put together the best and worst of each category so you can go keto with confidence. Saturated fats including coconut oil, ghee, grass-fed butter, duck fat, tallow, and lard-all essential for a healthy immune system, dense bones, and proper testosterone levels. And since they’re so delicious and protein-rich, you might find yourself asking: Are beans keto? Here’s the answer: It’s complicated. First things first, let’s get a little bit more specific about what the keto diet even is. “A keto diet is a very low carbohydrate diet,” explains Keri Gans, RDN. “The goal is to put your body into ketosis, and that is achieved by reducing your carb consumption to around 20 to 50 grams per day, depending on your size.” Overall, your diet is supposed to consist mainly of calories from fat, says Gans, which is ultimately quite restrictive compared to other eating methods.|Sugar-free sweeteners like allulose, erythritol, monk fruit and stevia. By all accounts, the “keto” diet, as it’s widely known, may even reverse type 2 diabetes, at least for some lucky individuals. Another advantage to the keto diet: It can help reduce systemic inflammation, which can have a variety of negative effects on your entire body. You can create meals that are appealing and delicious when following a keto diet. Unlike some of the other popular low-carb diets, which typically are high in animal protein, the keto diet focuses on getting to the body to burn stored body fat instead of sugar as the main fuel. When body fat is broken down in the liver instead of glucose, s an energy byproduct known as ketones are produced. A ketogenic diet requires that fat comprise 60 to 80 percent of your total calories. Protein makes up about 20 percent, while 10 percent comes from carbs. Generally speaking, it’s best to keep carb intake between 20-30 grams per day in order to maintain ketosis. That’s the equivalent of about half a medium bagel. If this sounds like Atkins, it’s close, but “ketogenic diets tend to be more severe in carb restriction and have a more moderate protein restriction,” says Spencer Nadolsky, D.O., author of The Fat Loss Prescription. Though you can eat bacon on a ketogenic diet, the rest of the spectrum is limited. Starchy vegetables like potatoes, corn, and squash are too high in carbs. Same with most fruits. Milk, beans, rice, pasta, bread: nope. However, bacon is still heavily processed and has been linked to an increased risk of cancer and heart disease, so you may not want to eat it at every meal. To stay as healthy as possible, keto dieters should eat plenty of low-carb vegetables like red bell pepper, kale and cauliflower.

While the vegan keto diet may benefit your health, it has some potential drawbacks. Vegan diets tend to be low in important nutrients, especially if not carefully planned. Because the vegan keto diet is more restrictive than normal vegan diets, it’s critical that those following it supplement with high-quality vitamins and minerals and plan their meals to ensure a nutritionally adequate diet. Eating fortified foods, focusing on whole-foods and enhancing nutrient availability, for example through fermenting and sprouting, is important for people following a vegan keto diet. However, it may be difficult for vegan keto dieters to meet their micronutrient needs through food alone. Supplementing with certain vitamins and minerals commonly lacking in vegan diets is a smart way to prevent potential deficiencies and ensure that your daily requirements are met. Sweet wines like riesling, moscato, sherry, and port can each have as many as 20g of carbs per glass. Sauces contain sugar and can be a bad idea on a keto diet. The dietitian said, however, that medical professionals are not sure why the diet works in these cases. “There is not a clear definition of what is happening,” she said. Rudy Mawer, a sports nutritionist, has also found some success with the keto type of diet. He said he uses this low-carb approach with some people who have trouble losing weight. He also has high performing athletes on the plan. Mawer told Healthline there are a number of benefits to the program. One benefit is its quick results. People can lose some initial weight rapidly and that, in turn, helps encourage them. “You can get motivated by this weight loss,” he said. He added the keto diet is simple in concept. It eliminates a food group, making it easier for people to follow. He said the diet also makes people feel full despite having fewer calories and it gives them more energy. That’s because, he said, people are giving up their sluggish diet of processed foods. He added the keto diet keeps blood sugar levels stable, which produces a more stable flow of energy. My friend says I should do a keto diet to help with my weight loss. What is it, and is it better for weight loss? A. A ketogenic (keto) diet is high in fat and protein and low in carbohydrates. Most of the body's cells prefer to use blood sugar (glucose) as their main source of energy. The keto diet forces your body to use a different type of fuel. Instead of providing your body with glucose from carbohydrates, the keto diet relies on the liver to break down stored fat into molecules called ketones. And the data we have so far suggest keto performs a lot like basically every other diet. There are some people who see tremendous success. There are some people who just absolutely miserably fail and maybe even gain weight on the diet. There are five variations of the Ketogenic Diet which have been published in medical literature as effective treatments for diseases that have an underlying metabolic dysregulation, such as epilepsy, cancer, and Alzheimer’s. The original Ketogenic Therapy, known as the classic Ketogenic Diet, or classic Keto for short, was designed in 1923 by Dr. Russell Wilder at the Mayo Clinic for the treatment of epilepsy. All Ketogenic Diets are a variation of classic Keto, which is the most strict, seen by it’s ratio of fat to protein and carbs, also called the macronutrient ratio. Classic Keto carries a 4:1 ratio, which means that there are four parts fat for every one part protein and carb. Since fat has a higher caloric content versus protein and carb (fat has 9 calories per gram, while both protein and carb have just 4 calories per gram), 90% of calories come from fat in a classic Ketogenic Diet, while 6% come from protein, and 4% come from carb. This process occurs when there isn’t enough glucose available to burn, so the body turns to stored fats instead. You are likely to realize that your body has achieved ketosis because you may have a dry mouth, increased thirst, and find yourself needing to urinate more frequently, Dr. Goss says. “You are ridding your body not only of ketones but electrolytes such as sodium, potassium, and magnesium,” which may lead to dehydration, she says, so drinking plenty of fluids will help reduce any problems even if it means more trips the bathroom. Ketoacidosis most commonly arises with people with diabetes. Diabetic ketoacidosis occurs in response to a severe lack of proper insulin activity whereas alcoholic ketoacidosis is a harmful metabolic condition arising when someone consumes alcohol but no food, explains Michael J. Gonzalez-Campoy, MD, PhD, medical director and CEO of the Minnesota Center for Obesity, Metabolism, and Endocrinology, in Eagan, Minnesota. Either way, this is a dangerous situation.
