A ketogenic diet, or keto for short, is a low-carb, high-fat diet that can provide a number of health benefits. It effectively turns your body into a fat-burning machine, while simultaneously helping you lose weight and boost your energy levels. How Does Keto Impact Weight Loss? Keto has some traits that are reminiscent of the Atkins diet and other low-carb plans. The keto diet involves dramatically reducing your carbohydrate consumption and substituting fat instead. This dietary alteration will put your body into a metabolic state known as ketosis. Ketosis is a completely normal metabolic function. Essentially, when your body doesn’t have enough glucose for energy, it will burn stored fat instead. Glucose is created when carbohydrates are broken down inside your body. Cutting carbs from your diet means less glucose for your body to burn up. Replacing carbs with fats will cause acids called ketones to build up in your body. The goal is to force the body to derive its fuel from fat, rather than carbs.|According to research, the traditional keto diet may offer benefits for certain health conditions, including obesity, metabolic syndrome, and diabetes. People use the keto approach to put their body into a state called ketosis. Under normal dietary conditions, the body breaks carbohydrates down into glucose, which it uses as its primary source of energy. When a person restricts their carbohydrate intake, the body produces ketone bodies to use for energy instead of glucose. Ketosis can cause someone to store less energy as fat and utilize fat for fuel, which may account for the effects of the keto diet on weight and metabolism. While restricting carbohydrate intake is the primary driver of ketosis, relative intakes of the other macronutrients matter, too. A person may not get into ketosis if they do not adhere to the traditional keto diet’s macronutrient ratios. For example, according to a review of studies, limiting protein intake to less than 1 gram per pound of body weight prevents glucose production in the body via gluconeogenesis. The average person doesn’t fully understand what ketosis really means, or that it’s necessary to measure blood levels of ketones. The mere presence of a few ketones doesn’t make one in ketosis. And peeing on a stick isn’t the way to measure blood levels of ketones, because simply spilling a few ketones in urine isn’t indicative of ketosis. After all, we all produce ketones after a certain period of not eating. That doesn’t mean that our body is magically shifting gears and the ketones are changing how we’re oxidizing all the food in our bodies. The ketogenic diet’s extreme restriction of carbohydrates contributes to micronutrient deficiencies and impacts gut health. Ketogenic diets don’t just eliminate sugar and refined carbohydrates; they also cut out pulses, whole grains, fruit, and starchy vegetables. All of these foods contribute vitamins, minerals, antioxidants, phytochemicals, and fiber which include prebiotic fiber that promotes a healthy gut microbiota.|“I don’t recommend staying on the keto diet permanently. Long-term research on the keto diet is limited, so there’s no telling for sure what cutting out certain major food groups and cutting carbs will eventually do to your body. That’s good, because the last thing you want to do is go back to your old eating habits and regain weight that you lost. While everyone’s nutrition requirements differ, generally, you may eat 40 to 45 percent fat and stick to 120 to 150 g of carbs per day to help you keep the weight off, she says. That said, the US News report noted that the long-term weight loss effects of keto aren’t clear. One concern voiced in some studies is loss of muscle tissue beyond what might be expected from any calorie-restricted weight loss diet, possibly because the body is attempting to break down protein to create glucose, but Volek says this isn't an issue when the ketogenic diet is followed correctly. Could the ketogenic diet's extreme restriction of carbohydrates contribute to micronutrient deficiencies and impact gut health? Ketogenic diets don't just eliminate sugar and refined carbohydrates; they also curtail pulses, whole grains, fruit, and starchy vegetables. The technical meaning of ketosis is the metabolic state in which your body accumulates ketones faster than they’re being burned. When we’re fat-adapted (a.k.a. ’re able to enjoy the benefits of burning fat and ketones as first-rung fuel sources. However, when we’re in ketosis without being keto-adapted (e.g. on a crash diet), we’re likely to excrete these ketones in our urine and breath instead of burning them for energy, which means we’re still left carb-dependent. As with any dietary pattern, the fats don’t have to be animal fats. A study found that a ketogenic diet high in polyunsaturated fats was superior to a saturated fat-rich ketogenic diet by several measures. Getting more plant-based fats from avocados, nuts, seeds, coconut, and olive oil will supply more micronutrients as well as heart-healthy fatty acids. Research on long-term effects of the ketogenic diet are currently lacking, longer-term research studies are in progress. Studying the ketogenic diet for a long time is more difficult than many diets because it is so extreme. Just because the ketogenic diet is trendy doesn’t mean everyone should hop on the bandwagon. There are other dietary patterns that are shown to promote health and a healthy weight, including Mediterranean and vegetarian dietary patterns. And research has demonstrated that some people thrive on lower-fat, higher-carb diets. It’s just a tool in the toolbox that people can implement.
Think of keto as a tool to make it much easier to maintain a calorie deficit. Even keto, as awesome as it is, isn’t a ‘eat as much as you want and still lose weight’ magic bullet. 1. When not in ketosis, your liver and muscles are full of glycogen, about 1-2 pounds worth depending on your body size. For every 1 part of glycogen, your body holds on to 3 parts of water (source). When we enter ketosis, our body will use up that glycogen and shed the associated 3 parts of water. 2. When you cut out major food groups, primarily sugar, grains, and gluten, your inflammation (mild swelling throughout the body) will go down. Most people are sensitive to these foods, and when we eliminate them we stop chronic inflammation. You can read more about inflammation and the body here. 3. When you eat a carbohydrate-rich diet, lots of water is required in your digestive tract to break down the carbohydrates. So when we eat a bagel or a peach at lunch, the glucose levels in our blood rise and the pancreas secretes insulin to turn glucose into a usable energy source, so it’s able to move the glucose from the blood into our cells. But our body didn’t evolve to actually keep glucose on storage so we just get it basically after we eat. There’s no long-lasting glucose stores around our body. So, if we stop eating carbs, our body has this amazing ability to adapt to whatever else we’re putting in there and turn that into a usable energy source. It’s kind of like you think about our bodies like flex-fuel vehicles. They can sort of adapt to whatever we’re putting in them. Like a hybrid car. Exactly. One of those adaptations is this process called ketogenesis. And when we’re in ketosis, the liver starts to break fat down both from the food we’re eating and from the fat that’s stored in our body into this usable energy source called ketone bodies, or ketones for short.|Additionally, the healthy fats that are being consumed as part of the diet provide a feeling of fullness, which results in less overeating. What foods can you eat while on the keto diet? Healthy fats - such as avocado and avocado oil, olive oil, and nuts and seeds - and some saturated fats - such as coconut oil, ghee, butter, and heavy whipping cream made from grass-fed cow’s milk - are all encouraged. Lean protein is allowed in specified amounts (too much protein can prevent ketosis). Poultry, lean beef, and cold-water fish (like salmon, herring, mackerel, and tuna) and other seafood are good protein sources. Of course, carbs are limited to dark green leafy vegetables, such as broccoli, spinach, and kale, as well as other low-carb veggies like cauliflower, peppers (red, yellow, and orange), asparagus, and zucchini. Berries are OK in small amounts. What foods should you avoid? Avoid simple sugars like sweetened beverages, juice, cakes, cookies, pasta, breads, cereals, and any carbohydrates that will trigger a rapid glucose increase. Switching over to a ketogenic diet can seem overwhelming, but it doesn’t have to be difficult. Your focus should be on reducing carbs while increasing the fat and protein content of meals and snacks. In order to reach and remain in a state of ketosis, carbs must be restricted. While certain people might only achieve ketosis by eating 20 grams of carbs per day, others may be successful with a much higher carb intake. Generally, the lower your carbohydrate intake, the easier it is to reach and stay in ketosis. This is why sticking to keto-friendly foods and avoiding items rich in carbs is the best way to successfully lose weight on a ketogenic diet. Eggs: Pastured, organic whole eggs make the best choice. Poultry: Chicken and turkey. Fatty fish: Wild-caught salmon, herring and mackerel. Meat: Grass-fed beef, venison, pork, organ meats and bison. Full-fat dairy: Yogurt, butter and cream. Protein is part of the keto diet, but it doesn't typically discriminate between lean protein foods and protein sources high in saturated fat such as beef, pork, and bacon. What about fruits and vegetables? All fruits are rich in carbs, but you can have certain fruits (usually berries) in small portions. Vegetables (also rich in carbs) are restricted to leafy greens (such as kale, Swiss chard, spinach), cauliflower, broccoli, Brussels sprouts, asparagus, bell peppers, onions, garlic, mushrooms, cucumber, celery, and summer squashes. A cup of chopped broccoli has about six carbs. A ketogenic diet has numerous risks. Top of the list: it's high in saturated fat. McManus recommends that you keep saturated fats to no more than 7% of your daily calories because of the link to heart disease. And indeed, the keto diet is associated with an increase in "bad" LDL cholesterol, which is also linked to heart disease. Nutrient deficiency. "If you're not eating a wide variety of vegetables, fruits, and grains, you may be at risk for deficiencies in micronutrients, including selenium, magnesium, phosphorus, and vitamins B and C," McManus says.|By following a keto diet plan, you force your body to burn fat, and you don’t switch back and forth, so your body stays in fat-burning mode all the time. Cutting back on high-fiber vegetables, fruits and whole grains also can increase risk for constipation. The keto diet has been used to help manage epilepsy, a disorder characterized by seizures, for more than 100 years. More recent studies are evaluating the keto diet as an alternative dietary treatment for obesity and diabetes. For me that comes out to about 1lb or 16oz of steak total, which has exactly the amount of protein I need to hit my ratios. If you’re smaller, it will likely be more like 12oz, which should be around the amount you need. This rule isn’t perfect, but it’s a simple starting point. Instead of constantly trying to figure out how much protein you’re getting, hold your fist up to it, and remember that you get two fists of fatty meat per day. You’ll get most of your fat from your fatty breakfast and your fatty meats, but you still need to add a bit more into each meal to make sure you hit your goal. Unfortunately, they ranked the Keto diet shockingly low, at number 37. The Low-Carb Action Network (L-CAN) wonders why US News would cite concerns about nutritional imbalances and sustainability, when there are over 65 clinical trials proving its effectiveness. Indeed, L-CAN has a page dedicated to keto and low-carb diet nutritional adequacy. In fact, this is not the first time US News has shown bias against a low-carb or keto diet. In March, the magazine published an article criticizing the Department of Veteran Affairs (VA) for launching a program that uses a ketogenic diet combined with a mobile app to treat Type 2 diabetes, in partnership with a private company, Virta Health. According to the authors, this may be dependent on circulating ketone levels, but a threshold level had yet to be determined. Research also shows benefits for improving insulin sensitivity and glycemic control, although the mechanisms aren't clear.1 A 12-month study that randomized 34 adults with HbA1c levels above 6% and BMIs above 25 to a very low-carb ketogenic diet or a moderate-carb, calorie-restricted, low-fat diet found that the low-carb group had greater weight loss and reduction of HbA1c. The low-carb subjects also were more likely to be able to reduce use of diabetes medications, with three discontinuing metformin. Studies-most of which compare ketogenic diets with low-fat diets-also have found that while LDL cholesterol tends to increase for some people following a ketogenic diet, small, dense LDLs tend to decrease.1,13,14 Triglyceride and HDL cholesterol levels also tend to improve. And although it's still controversial among dietitians, doctors, and other nutrition experts, there is some scientific research to back it up. Avoid processed and ultra-processed foods high in calories and bankrupt in terms of nutrients: those made with white flour or wheat flour products, added table sugar, conventional dairy, bread and other processed grains like pasta, sweetened snacks like cookies and cakes, most boxed cereals, sweetened drinks, ice cream and pizza. Remember, the ketogenic diet will actually change your metabolism, put you into ketosis and turn you from a sugar burner to a fat burner. Those are significant changes for your body, and you’re bound to notice some symptoms of the so-called keto flu. Keto flu symptoms and side effects can include feeling tired, having difficulty sleeping, digestive issues like constipation, weakness during workouts, being moody, losing libido and having bad breath. Fortunately, these side effects don’t affect everyone and often only last for 1-2 weeks. Overall, symptoms go away as your body adjusts to being in ketosis. If a ketogenic diet is being used for a child to treat epilepsy, close medical monitoring is necessary. If you’re very active and without much body fat, consider trying carb cycling or at least eating a modified keto diet that does not severely restrict carb intake. Ketogenic diets were originally developed to help improve symptoms of epilepsy (specifically in children who didn’t improve from other treatments), but today very low-carb diets are used to help adults, too, including those suffering from many other chronic health problems like obesity, cancer and diabetes. Does the keto diet work? Yes! Rapid and reliable weight loss will occur in even a keto for beginners diet, due to lowered insulin levels and the body being forced to burn stored body fat for energy. Not everyone will feel best eating high fat and many will not lose their body fat eating that much on a daily basis. Your activity level, lifestyle, age, and genetics, are all factors in what will help you achieve weight loss. We are all bio-individual and have different needs in regards to daily macros. But one thing remains the same, for a well-balanced, ketogenic diet that promotes health and fat loss, eating whole, single ingredient, nutrient dense foods is the best way to achieve your health goals. So if your goal is weight loss, eat enough fat to be satisfied, but there’s no need to “get your fats in” once you’re satisfied. Drink lots of water. This is especially crucial on a low carb or keto diet. Why? When you eat carbohydrates, your body stores the extra as glycogen in the liver, where they are bound to water molecules. Eating low carb depletes this glycogen, which allows you to burn fat - but it also means you are storing less water, making it easier to get dehydrated. Instead of the traditional recommendation of 8 cups of water per day, aim for 16 cups when following a low carb lifestyle. Keep up electrolytes. The major electrolytes in our bodies are sodium, potassium and magnesium. Because a low carb diet (especially a keto diet!) reduces the amount of water you store, this can flush out electrolytes and make you feel sick (called “keto flu”). Caffeine is fine for most people-just don’t go pouring in sugar or milk; the same goes for tea and nut milk. Lower-carb alcohol in moderation is OK, especially if you’re at the point where you’re just trying to maintain weight. Soft drinks, fruit juices, sweet wines, craft beers, and flavored liquor are filled with way too much sugar and/or carbs to be allowed if you’re serious about keto. Some people will drink diet, or “zero,” soft drinks, but avoid them if you can because the citric acid and aspartame often found in them may derail your trip to ketosis. Sweeteners like stevia, erythritol, and xylitol can be made a part of your keto diet, but try to buy only the pure versions, as the powdered products usually have a small amount of sugar added as a bulking agent. Inulin is a sweet and starchy plant fiber that helps regulate blood sugar. Monk fruit powder is 300 times sweeter than sugar and doesn’t have a bitter aftertaste like stevia. 70% cocoa dark chocolate and cocoa powder are packed with antioxidants. Sugar, high-fructose corn syrup, honey, and agave nectar need to be ditched. Even if honey and agave are healthy whole foods, sugar is still sugar and will bump you out of ketosis. Unlike the Atkins Diet, which is high in protein, a keto diet avoids eating too much protein. In the past, people thought large amounts of protein can turn into glucose in a process called gluconeogenesis, which takes you out of ketosis. More recent findings suggest the possibility of gluconeogenesis isn’t as big a problem as we once believed, but on a keto diet, the majority of your calories should come from quality fats-not protein. There’s a bit of a learning curve when you’re finding out what to eat on keto. Broadly speaking, it’s best to get your dietary fat from nutrient-dense, whole food sources. That means eating more foods like avocados, coconut oil, olive oil and butter (or Bulletproof Grass-Fed Ghee). Your protein intake should primarily come from fatty cuts of protein like salmon and, yes, bacon. How long does it take to get into ketosis? It can take anywhere from 2-3 days to a few weeks to enter ketosis, depending on your body’s ability to adapt to burning fat for fuel and increasing your ketone levels. If you’re insulin resistant, you might need to start lower, around 20 to 30 grams. Hard-charging endurance athletes can often eat 100 grams or more per day and still achieve ketosis. Everyone approaches keto dieting a bit differently. I recommend tracking total carbs instead of net carbs for ease of tracking, and I don’t worry about the carbs in above-ground green vegetables and avocados. What is the difference between total carbs and net carbs? Total carbs is just what it sounds like: all the carbohydrates contained in a given food. Net carbs is the total carbohydrate minus any fiber. Some keto plans count net carbs because fiber is not metabolized into glucose, so it doesn’t affect insulin levels or ketosis. How to calculate net carbs on keto? Phase 1 (induction): Under 20 grams of carbs per day for 2 weeks. Eat high-fat, high-protein, with low-carb vegetables like leafy greens. This kick-starts the weight loss. Phase 2 (balancing): Slowly add more nuts, low-carb vegetables and small amounts of fruit back to your diet. Phase 3 (fine-tuning): When you’re very close to your goal weight, add more carbs to your diet until weight loss slows down. Phase 4 (maintenance): Here you can eat as many healthy carbs as your body can tolerate without regaining weight. However, these phases are a bit complicated and may not be necessary. You should be able to lose weight and keep it off as long as you stick to the meal plan below. Some people choose to skip the induction phase altogether and include plenty of vegetables and fruit from the start. This approach can be very effective as well. Others prefer to just stay in the induction phase indefinitely. This is also known as a very low-carb ketogenic diet (keto). Our hunter-gatherer ancestors didn’t have regular access to much carb intake beyond seasonal fruits or tubers. They spent much of the time operating in ketosis-the metabolic state of producing ketones as a fuel source in the context of carb restriction. Our bodies today fully retain this ability, and there are substantial benefits from a lifestyle that incorporates that full metabolic flexibility. What Is the Keto Reset Diet? Do NOT WORRY about macros during your first week. It can become overwhelming really quickly. Cut out high glycemic foods like bread, pasta, and rice, but believe it or not, because there are so many great substitutions you won't miss these starchy foods. And by substituting these foods with high fat low carb foods, you won't get hungry. Eat lots of non-starchy veggies like cauliflower, broccoli, and green leafy vegetables. Your proteins will be moderate, with only 15 to 30% of your total intake coming from healthy proteins like chicken, beef, fish or tofu. Fats will make up the rest of your daily intake. That means the diet has been touted as a treatment for hyperthyroidism; nonetheless, the approach is controversial. In one small 2013 study of adults without diabetes with BMIs in the overweight or obese range, levels of ghrelin (the hunger hormone) and subjective appetite were lower when the subjects were in ketosis, even on a very low-energy diet (VLED) of 500 to 550 kcal per day. However, after eight weeks on the VLED, subjects began refeeding, and, at week 10, ratings of hunger and the urge and desire to eat were significantly higher than before their weight loss. I rarely suggest the ketogenic diet to clients, if they come in wanting to try the diet, I recommend a full nutrition assessment. This includes evaluating any risk factors that might indicate the ketogenic diet isn’t safe for them, such as individuals who are pregnant, there’s a lack of research on the ketogenic diet during pregnancy. I want to make it clear that the ketogenic diet isn’t the only way to lose weight.|The ketogenic (or keto) diet is a rapidly growing dietary trend for weight loss and was initially designed in the USA in the 1920s for treatment of refractory epilepsy. It was a diet that resulted in similar physiological effects to that of fasting, which appeared to reduce the incidence of epileptic seizures, but it could be maintained for a longer time. Since the 1990s, the keto diet has been under scrutiny for its use in the treatment of people with obesity or who are overweight, metabolic syndrome, cancers, and specific psychiatric and neurological disorders.
