Thus, baseline and follow-up lipid/lipoprotein assessments are recommended for individuals choosing to follow these diets, said Kirkpatrick and Maki. It’s also important to note that very low-carb or keto diets are not for everyone. People with type 2 diabetes may experience hypoglycemia (low blood sugar) if they don’t adjust their medication, and those who take vitamin K-dependent anticoagulants may need more frequent monitoring due to a potential change in vitamin K intake. At times emphasis has been put on a single new aspect that the previous diets had overlooked and the new one was to embrace in order to improve weight loss and well-being. Unfortunately, very few randomized clinical trials involving diets have addressed the combined question of weight loss and cardiovascular outcomes. The recently introduced ketogenic diet requires a rigorous limitation of carbohydrates while allowing a liberal ingestion of fats (including saturated fats) and has generated a flurry of interest with many taking the pro position and as many taking the cons position. The ketogenic diet causes a rapid and sensible weight loss along with favourable biomarker changes, such as a reduction in serum hemoglobin A1c in patients with diabetes mellitus type 2. However, it also causes a substantial rise in low density lipoprotein cholesterol levels and many physicians are therefore hesitant to endorse it. In view of the popular uptake of the keto diet even among subjects not in need of weight loss, there is some preoccupation with the potential long-term consequences of a wide embrace of this diet by large segments of the population. On the contrary, numerous lines of evidence show that plant-based diets are associated with reduction in oncological and cardiovascular diseases and a prolonged life span. The debate reproduced in this article took place during a continuous medical education program between two cardiologists with largely differing views on the matter of effectiveness, sustainability, and safety of the ketogenic diet compared to alternative options.|Though it sounds simple enough, the process involves many biological modifications and it may take a few weeks for these changes to occur. But it can be risky for certain folks, like those with kidney issues, according to experts at the Keck School of Medicine of USC. Ninety percent of Americans have been on a diet and when you ask why they’re dieting - is it for health or to lose weight? “the predominant answer is to lose weight,” according to Dean Pinchas Cohen of the USC Leonard Davis School of Gerontology. So it’s no surprise keto - essentially a new take on the Atkins diet - is popular since it results in weight loss, he said. But long term, the keto diet could be detrimental, since cutting out carbs increases health risks and mortality over time, Cohen said. The ketogenic diet is usually something that’s prescribed by a registered dietitian. Though carbs should be restricted, low-glycemic fruits such as berries can be enjoyed in limited amounts as long as you’re maintaining a keto-friendly macronutrient range. Be sure to choose healthy food sources and steer clear of processed foods and unhealthy fats. Unhealthy fats: Margarine, shortening and vegetable oils such as canola and corn oil. Processed foods: Fast food, packaged foods and processed meats such as hot dogs and lunch meats. Diet foods: Foods that contain artificial colors, preservatives and sweeteners such as sugar alcohols and aspartame. Sugar can be found in a wide variety of beverages including juice, soda, iced tea and coffee drinks. While on a ketogenic diet, high-carb drinks must be avoided just like high-carb foods. Thankfully, there are many tasty, sugar-free options for those on the keto diet. Water: Water is the best choice for hydration and should be consumed throughout the day. Sparkling water: Sparkling water can make an excellent soda replacement. Unsweetened coffee: Try heavy cream to add flavor to your cup of joe. Unsweetened green tea: Green tea is delicious and provides many health benefits. If you want to add some extra flavor to your water, try experimenting with different keto-friendly flavor combinations. For example, tossing some fresh mint and lemon peel into your water bottle can make hydration a breeze. Though alcohol should be restricted, enjoying a low-carb drink like vodka or tequila mixed with soda water is perfectly fine on occasion. A healthy ketogenic diet should revolve around high-fat, low-carb food choices and restrict highly processed items and unhealthy fats. Keto-friendly beverage options must be sugar-free. Consider water, sparkling water or unsweetened green tea and coffee.
Calcium is also lacking. Most Ketogenic diet enthusiasts remain unaware that complex carbohydrates contain hundreds of beneficial substances called phytochemicals; which are naturally occurring non-nutrients that help to prevent disease. Not a long-term solution. The weight loss from the Ketogenic approach is typically temporary, not permanent. When you change your eating habits short-term, your weight will also change short-term. Unfortunately, more and more studies suggest that low-carb diets may actually increase your risk for all-cause mortality, as well as cardiovascular disease and cancer mortality. For the majority of the population though, the strict macro tracking, dietary limitations, and unpleasant side effects are enough reason to drop this fad and aim for making healthier lifestyle changes. It came from a group of Chinese researchers who presented a study at the American College of Cardiology annual meeting in New Orleans. They found patients who followed a diet low in carbohydrates for nearly two decades had an increased risk for atrial fibrillation. But there are important caveats to consider. While the study suggests a link between the two, it wasn’t a randomized trial, so it doesn’t show a cause and effect, and the participants weren’t on the keto diet. Does It Work For Anyone? While Lisa says there's no reason for the average person to try the keto diet - "science supporting the ketogenic diet is either widely discredited or nonexistent" - it can work for children with epilepsy or someone who has suffered brain trauma or injury. There is some science to back up how a keto diet would work in those instances, but Lisa warns that those people should be under the supervision of both a doctor and a dietitian. Consider where you're getting your information on keto. Is it from social media? Does your source have a degree? Are they versed in nutrition? Make sure you're getting the best information possible - this is your health we're talking about, after all! In Summary . . Lisa said. We're happy to hear it, because we also really like pasta. She warns not to try this (or any version of keto) before talking with a physician if you have diabetes and are insulin dependent, as it could lead to a too-low blood sugar level. How It Works This version of keto calls for upping the protein intake just a bit. Protein should make up about 30 percent of calories, with the other 65 percent coming from fat and 5 percent from carbs, Spritzler says. Aim to source your protein from both animals (meat, fish, and dairy) and plants (nuts and seeds), Spritzler suggests. Although this diet may be beneficial for some people, there are side effects you may encounter a few days into eating keto. For many people, these symptoms only last a few days, not like a real flu. However, if they continue past that or you don’t feel well, listen to your body and stop the diet. These symptoms are a sign that your body is transitioning and getting used to being in ketosis. To combat these symptoms (or minimize them), easing into the diet might be a good place to start. You could try carb cycling two to three days per week until you’re fully ready to approach the keto diet. Some people end up sticking with carb cycling because they feel it’s beneficial for them. But everyone is different - so just be mindful of your body and what works best for you. As you increase your water intake and drop water weight, your electrolytes may swing out of balance and increase the intensity of your symptoms. To counter this, drink bone broth for sodium, eat sliced dried apricots with yogurt for potassium, or a piece of dark chocolate for magnesium. And ketosis, the process by which the body uses those fuels, is essential for survival. Here’s how it works: The body-even that of a very lean athlete- stores about 40,000 calories of fat compared with just 2,000 calories of the carbohydrate glycogen. When those carbs have been depleted, the body taps its fat stores for energy. The same is true for athletes who “bonk” during exercise-it’s because they’ve used up all their stored carbs. To go on, they must either eat more carbs (to burn as sugar) or start burning fat. When marathoners break through the so-called “wall” late in a race, they’ve begun to burn fat. ESSENTIALLY ALL legumes, dairy, refined sugar, potatoes, processed foods, refined vegetable oils, and salt (yes, you read that right), are off-limits on the Paleo Diet. Grass-fed meat, seafood, fresh fruits and vegetables, eggs, nuts and seeds, and certain oils like coconut, avocado, and olive oils. A paleo-friendly dinner might include grilled chicken with steamed vegetables, avocado and fruit. If you think paleo is rigid, keto is even less flexible. Generally speaking, dieters are advised to eat between 20 to 30 grams of carbohydrates per day in order to maintain ketosis. To put this into perspective, a quarter cup of steel cut oats has 29 grams of carbs and a banana has roughly 27 grams of carbs. So if you have a few bites of oatmeal or a small piece of fruit, whoops. That's your carb intake for the day.
There has been some speculation that higher dietary protein intake may prevent nutritional ketosis, but the literature does not support this. However, when you examine the data across all studies examining protein intake and blood ketone levels, there is no strong correlation. It appears as those dietary protein does not substantially affect blood ketone levels. One can also increase blood ketone levels through the use of supplementation. For example, one can consume exogenous ketones or high amounts of medium-chain triglycerides to increase blood ketone levels to a state of nutritional ketosis. So when we eat a bagel or a peach at lunch, the glucose levels in our blood rise and the pancreas secretes insulin to turn glucose into a usable energy source, so it’s able to move the glucose from the blood into our cells. But our body didn’t evolve to actually keep glucose on storage so we just get it basically after we eat. There’s no long-lasting glucose stores around our body. So, if we stop eating carbs, our body has this amazing ability to adapt to whatever else we’re putting in there and turn that into a usable energy source. It’s kind of like you think about our bodies like flex-fuel vehicles. They can sort of adapt to whatever we’re putting in them. Like a hybrid car. Exactly. One of those adaptations is this process called ketogenesis. And when we’re in ketosis, the liver starts to break fat down both from the food we’re eating and from the fat that’s stored in our body into this usable energy source called ketone bodies, or ketones for short.|Additionally, the healthy fats that are being consumed as part of the diet provide a feeling of fullness, which results in less overeating. What foods can you eat while on the keto diet? Healthy fats - such as avocado and avocado oil, olive oil, and nuts and seeds - and some saturated fats - such as coconut oil, ghee, butter, and heavy whipping cream made from grass-fed cow’s milk - are all encouraged. Lean protein is allowed in specified amounts (too much protein can prevent ketosis). Poultry, lean beef, and cold-water fish (like salmon, herring, mackerel, and tuna) and other seafood are good protein sources. Of course, carbs are limited to dark green leafy vegetables, such as broccoli, spinach, and kale, as well as other low-carb veggies like cauliflower, peppers (red, yellow, and orange), asparagus, and zucchini. Berries are OK in small amounts. What foods should you avoid? Avoid simple sugars like sweetened beverages, juice, cakes, cookies, pasta, breads, cereals, and any carbohydrates that will trigger a rapid glucose increase. Unfortunately, keto diets are probably more prone than many others to end with weight regain because they can be hard to stick to in the long run, Carson said. And being in ketosis for more than a few weeks might not be best for overall health, she said. Some of this is genetically determined: The extent to which dietary fats and cholesterol translate to increased levels of blood cholesterol (which are associated with heart disease) is partly individual, Carson said. Ketogenic diets also tend to cause more calcium to be lost in the urine, Carson said, which can lead to a decrease in bone density over time and increase the risk of osteoporosis. The AHA recommends eating a variety of fruits, vegetables, whole grains, low-fat dairy, skinless poultry and non-fried fish, nuts and legumes, and limiting red meat, saturated fat and sweets for a heart-healthy diet. Majumdar said. In the case of keto diets, putting the kibosh on fruits, many veggies and whole grains means that people don't end up consuming much fiber. The AHA also recommends at least 150 minutes of moderate exercise a week for heart health. Weight loss and maintenance might take a lot more exercise than that, Carson said, which could be a problem for those on ketogenic diets. “Success doesn’t happen overnight, and I know that working hard will eventually get me to my goals,” says Lele. And when it comes to finding motivation, being a mother certainly helps: “My son helps me stay on track. I need to get healthier for my son to make sure I’m always here for him,” she says. “Definitely try to stick with a ‘whole foods’ concept on keto,” says Lele. “Try your best to avoid convenience food. I understand that life can be busy, and that a protein bar or processed food is really tempting at first. Lele says that it’s important to remember that, while keto is a “high fat” diet, the goal is to use your body fat as an energy source, not the fat that’s on your plate. “You don’t need to necessarily add more fats to your diet to adhere to keto. For instance, if your dinner consists of avocado, bacon, and eggs, you really don’t need to add butter to that to make it ‘more keto’,” she says.|“A well-formulated keto diet includes all the food groups and therefore provides adequate intake of micronutrients,” says Dr. Goss. She adds that the research supports the safety and effectiveness of following a keto diet even in older adults with obesity as it seems to help improve body composition and improve metabolic health.6 “We saw significant reduction in body fat while lean mass was preserved and significant improvements in insulin sensitivity and the lipid profile in response to a ketogenic diet when compared to a standard ‘balanced’ diet,” Dr. Goss explains. Anyone who takes medications for diabetes or hypertension should consult with a physician before starting on the keto diet, she adds. And it is not for everyone: Individuals who are dependent on insulin (those with type 1 diabetes) should never follow the keto diet, says Dr. Goss. Additionally, since ketosis can be harmful to a fetus, the keto diet should be avoided during pregnancy. “Also, ketones are harmful to patients who cannot correct the metabolic acidosis caused by them, ie, those with liver, kidney, or pulmonary disease,” Dr. Gonzalez-Campos adds. If you decide to follow the diet, you’ll want to have an individualized meal plan, stresses Dr. Gonzalez-Campos. “The best possible clinical outcome is for each individual to meet her nutritional needs from well-balanced meals,” he says. Well, while they have roughly the same nutritional value as any other type of beans, green beans and black soybeans are particularly low in carbs: “A 1/2 cup of black soybeans provides only eight grams of carbs, 11 grams of protein, seven grams of fiber, and six grams total of fat,” Gans notes. Each of these nutritional values are determined based on half cup measurements. What are some good bean substitutes? Now that you have all the information you need on beans and their relationship to the keto diet, you might be wondering how you can work other low-carb, similarly-textured foods into your everyday eats and meal prep. (Or, if you just don’t like beans in general, you might be looking for some substitutes.) Here are some great go-to options, according to Sheth. Mushrooms. Because of their textured and earth-y taste, chopped mushrooms are a great substitute for any bean-centered meals if you’re looking for a swappable option. Yep, boiled peanuts are legumes, and they can totally be a go-to if you’re looking for a bean substitute. Why? “They are low in carbs and can provide a similar texture to dishes,” says Sheth. Avocado. Looking for a high-fat, low-carb substitute for your favorite beans? Avocados or dips like guacamole are a flavorful and easy-to-incorporate option, Sheth says. Ground meat. High-protein, lean ground meats are also a flavorful and filling substitute for beans, if you’re not on a vegetarian or vegan diet.|The ketogenic diet (also referred to as the keto diet) has been around for decades, but recently it’s grabbing headlines for its ability to enhance performance and melt pounds. But are these claims legit? And, more important, is it safe? “I get asked all of the time ‘What is the keto diet” followed by ‘Do you think I should do it,’” says Kelly Nohl, a registered dietitian with Henry Ford Health System. Here, Nohl answers those questions - and many more - so there’s no need to wonder about this popular diet plan anymore. Q: What is the ketogenic diet? A: The ketogenic diet (also called keto) dates back to the 1920s, when doctors began using it to control seizures among patients with epilepsy. The diet’s composition of carbohydrates, fat and protein force the body to use fat instead of carbohydrates for energy. Dubbed ketosis, this process creates acids in the blood called ketones that our bodies and brains use for fuel. Start with the easiest fasting window of 8-10 hours between dinner and breakfast. Track your carb intake. Carb restriction lowers blood sugar and insulin levels, leading to the release of ketones. Make sure to stick to the recommended daily intake of carbs to enter ketosis and get all the benefits of this metabolic state. Try to add more healthy fats into your daily routine instead of eating saturated fats and highly-processed foods. 75% of your daily caloric intake should come from fat, as it will boost your ketone levels. Focus on healthy fats like chicken broth, olive oil, ground beef, butter, avocado, avocado oil, nuts, flax seeds, hemp hearts, chia seeds, olives, fatty fish, whole eggs, and coconut oil. Avoid eating artificial trans fats (cakes, cookies, pastries, biscuits, crackers), processed meats (deli meat, sausages, salami, hot dogs, and cured and smoked meats) as well as fried food. Use MCT oil regularly. The medium-chain triglyceride fat in MCT oil will help you raise the ketones level in your body.
Grains and legumes contribute a significant amount of carbs to the diet. If you eat them while following the keto diet, you risk throwing your body out of ketosis. Keto and paleo diets strongly discourage the intake of added sugars. For both diet plans, this largely falls under their shared message of avoiding heavily processed foods in general. However, paleo dieters are a bit more flexible with this rule, as unrefined sugar sources like honey and maple syrup are still permitted. Keto, on the other hand, doesn’t allow any added sugar sources, refined or not, due to the high carb content of these foods. In line with their shared goal of achieving optimal health, both paleo and keto diets encourage the intake of unrefined, healthy fats. Both diets also recommend moderate-to-liberal amounts of selected refined oils, such as olive and avocado oils, as well as nuts, seeds and fish. Keto places very heavy emphasis on fat in general, as it is the cornerstone of the entire diet. Paleo, while not necessarily a high-fat diet, uses this recommendation to support overall health. One of the primary reasons for the popularity of keto and paleo diets is the notion that they will promote weight loss. Unfortunately, there is limited research available for how effective these diets are for sustained, long-term weight loss. However, some short-term research is promising. A small study of postmenopausal, obese women following the paleo diet showed a 9% weight loss after six months and a 10.6% loss at 12 months. This may have been because a high intake of fat usually leads to a decrease in appetite and fewer overall calories consumed. It may also be that the process of ketosis is leading to more efficient elimination of the body’s fat stores. The exact reason is still unclear. All meals should be planned in advance, evaluating the nutritional value of each of them. It is important not to deprive your body of fiber. Fiber is extremely important for intestinal health and can help you get into ketosis without experiencing symptoms of keto flu. If you can’t rigidly track your carb intake, you can try a gradual elimination method. Remove onethe source of carbohydrates from your diet every week. 1. Remove all desserts and sweet snacks such as cakes, pastries, and chocolate bars. 2. The third week is the time to say goodbye to starchy foods like pasta, pizza and packaged snacks. 3. Remove all bread, rolls and starchy vegetables. 4. By Week 5 you are probably down to 50 g of carbs a day. If you want to restrict carbohydrates further, then cut out fruit and sauces that contain carbs. Looking for a way to break the vicious cycle of weight loss and tone up all the jiggly parts? It’s not recommended that people who have elevated blood levels of cholesterol try the diet, and those who have atherosclerotic cardiovascular disease, a history of atrial fibrillation, or the presence or history of heart failure, kidney disease, or liver disease should talk with their doctor before trying the diet. On the flip side, low and very low-carb diets lowered triglyceride levels in study participants compared to those following high-carb, low-fat diets. And while these low-carb diets increased HDL (good) cholesterol levels in the short term, the beneficial effect diminished after six months or longer, especially in people with type 2 or pre-diabetes, said Kirkpatrick and Maki. There was also a reduction in the use of diabetes medication when people with type 2 diabetes followed low-carb diets, but the carb intake was not low enough to be considered a keto diet. Want to start running? While fitness or physical activity performance was not reviewed for this statement, current evidence does not support that very low-carb or ketogenic diets are more beneficial for fitness or physical activity performance in either recreational or elite athletes and, in fact, have resulted in decreased performance in some athletes, explained Kirkpatrick and Maki. The ideal dietary pattern to promote weight loss, as well as cardiovascular health, fitness, and general health depends on the person striving to lose weight. It’s important to take into consideration personal preferences and behavioral, family, cultural, and social dynamics, as well as ethnic or economic influences, the researchers added. Some people use the keto diet to stay at a healthy weight. Unlike glucose, ketones can’t be stored as fat because they aren’t metabolized the same way. This might seem counterintuitive if you associate keto with piles of bacon and cheese. But in reality, the keto diet can support weight management by burning fat and curbing cravings. The trick is to primarily get your fats from quality sources like nutrient-dense whole foods and pay attention to how you feel. Inflammation is your body’s natural response to an invader it deems harmful. Too much inflammation is bad news because it increases your risk of health problems. A keto diet can reduce inflammation in the body by switching off inflammatory pathways and producing fewer free radicals compared to glucose. What is Metabolic Flexibility and How Can You Achieve It? The keto diet for beginners seems like all fat, no carbs and lots of bacon and cheese-but that’s not the case. There are different approaches to this style of eating, and it’s a good idea to find what works for you.|The keto diet, short for "ketogenic," involves eating a high amount of fat, a moderate amount of protein and very few carbs - even fruit is off the table. As with any fad diet, adherents tout weight loss, increased energy and greater mental clarity among the benefits. But is the keto diet all it's cracked up to be? Not precisely, nutritionists and dietitians say. Low-carb diets like the keto do appear to lead to some short-term weight loss, but they're not significantly more effective than any other commercial or self-help diet. And as with other fad diets, people typically regain the weight once they go off the diet. So, what is the keto diet? The keto diet was originally designed not for weight loss, but for epilepsy. In the 1920s, doctors realized that keeping their patients on low-carb diets forced their bodies to use fat as the first-line source of fuel, instead of the usual glucose.
In response the criticism and misstatements in the article, some 300 health-care professionals-200 of them doctors- from around the world recently signed a response letter written by the Low-carb Action Network, which welcomes skepticism yet wonders why the U.S. News editorial team has been so excessively critical of a new, evidence-based approach that is strongly supported by rigorous, clinical trial evidence and, in the view of many doctors working on the front lines dealing with diet-related diseases, offers the first truly promising solution to a long-tragic epidemic. Unfortunately, it’s not just US News that appears to have bias against a low-carb or keto diet. In fact, CNN Health recently published an article questioning the sustainability of keto diets. “Absent ketosis, keto is just a false label for some kind of diet that presumably restricts added sugar and refined carbohydrate - which, frankly, any good diet does,” Katz said. That is incorrect. This way of eating is a means to reduce hyperinsulinemia which is driving the majority of modern chronic disease. If you’re trying out the keto diet you should take care to avoid sugary, starchy foods. Remember- carbohydrates should make up less than 5% of your entire calorie intake. Aim to have an absolute maximum of 50 grams of net carbs a day, but aim for 20 grams or less. Are there any ketogenic diet side effects? Immediately after starting the diet during the first week, some dieters may experience what is referred to as the “keto flu“. The keto flu is a term colloquially used to describe the general feelings of malaise some dieters feel at the beginning of the low-carb high-fat diet. Keto flu can be prevented or helped by focusing on getting plenty of sleep, increasing electrolyte intake, and reducing consumption of dehydrating drinks like caffeine and alcohol. But fear not! Starting out with keto has some positive benefits, too. During the initial adjustment phase most dieters report rapid weight loss - losing up to 10 lbs of body weight in 2 weeks or less. Among the five who completed the study, they lost 12 percent of their weight on average and reduced fasting insulin by 54 percent. Additionally, two women who previously experienced infertility problems became pregnant. This process of burning fat provides more benefits than simply helping us to shed extra weight - it also helps control the release of hormones like insulin, which plays a role in development of diabetes and other health problems. When we eat carbohydrates, insulin is released as a reaction to elevated blood glucose (an increase in sugar circulating in our blood) and insulin levels rise. Insulin is a “storage hormone” that signals cells to store as much available energy as possible, initially as glycogen (aka stored carbohydrates in our muscles) and then as body fat. The keto diet works by eliminating carbohydrates from the your daily intake and keeping the body’s carbohydrate stores almost empty, therefore preventing too much insulin from being released following food consumption and creating normal blood sugar levels.|4. Once the blood levels of ketones rise to a certain point, you officially reach ketosis. 5. This state results in consistent, fairly quick weight loss until your body reaches a healthy and stable weight. What Is the Keto Diet? What is the ketogenic diet exactly? The classic ketogenic diet is a very low-carb diet plan that was originally designed in the 1920s for patients with epilepsy by researchers working at Johns Hopkins Medical Center. Researchers found that fasting - avoiding consumption of all foods for a brief period of time (such as with intermittent fasting), including those that provide carbohydrates - helped reduce the amount of seizures patients suffered, in addition to having other positive effects on body fat, blood sugar, cholesterol and hunger levels. Unfortunately, long-term fasting is not a feasible option for more than a few days, therefore the keto diet was developed to mimic the same beneficial effects of fasting. After 20 years of January diets, my own experience suggested that nothing made any difference: you only lost weight if you took in less calories than you used, and if you took in less calories than you used then you were hungry. Simple as that. Could it be that a ketogenic diet really was different? Well, the principle makes sense. Bizarrely named “ketone bodies” are actually molecules that act as the body’s natural back-up fuel supply when glucose is scarce. Normally, we only enter ketosis (where ketone bodies accumulate in the blood) when we starve ourselves - not just overnight or by missing a meal, but for several days at time. Can you have cheat days on keto? “Cheat meals”-which I take to mean eating foods like desserts, bakery items, pizza, and the like-will quickly put you over your carb allotment for the day.
